School of Basic Midwifery UDUTH Form 2024/2025
Applications are invited from suitably qualified candidates for admission into the School of Basic Midwifery, Usmanu Danfodiyo University Teaching Hospital Sokoto, for the Basic Midwifery Programme 2024/2025 academic session.
Applicants with all the following qualifications may be considered for admission into the School of Midwifery:
- Must be at least 16 years at the time of application.
- Possess at least five (5) Credits in WASC, SSCE, GCE, NABTEB or NECO which should include Physics, Chemistry, Biology, Mathematics and English Language at not more than two (2) sittings.
Duration: The duration of the program (Basic Midwifery Education) program shall be two (3) academic sessions of Six (6) semesters.

How to Apply for UDUTH School of Midwifery Admission Form
School of Basic Midwifery, Usmanu Danfodiyo University Teaching Hospital hereby informs all interested applicants to visit the application portal at spbm.uduth.org.ng/basic in order to participate in the online application exercise for the 2024/2025 Academic session.
- A non-refundable fee of Five Thousand Naira (N5,000) is required. This can be transacted via Remita.
- On the Remita platform, choose “Usmanu Danfodiyo University Teaching Hospital Sokoto” from the MDA Remita payment dropdown.
- For services, select “UDUTH School of Midwifery.” Ensure that N5,000 is the service amount and complete the other necessary details before selecting “Generate Payment Instruction.”
- With the provided Remita (RRR Number), proceed to any bank for payment.
- Post payment, the bank will provide a teller. Present this, along with the Remita Slip, to the SOM-UDUTH cashier to procure a receipt.
- This receipt and a copy of the Remita Slip should be presented at the SCHOOL OF MIDWIFERY UDUTH SOKOTO to acquire your unique Serial Number and Pin.
- With these credentials, access the application portal at spbm.uduth.org.ng/basic. Enter your details with utmost precision, and subsequently submit your form.
- Retain a printed copy of this filled form as it is mandatory during the Entrance Exams.
