- Two recent passport photographs.
- Original SSCE/NECO Certificates or a printed copy of their WAEC/NECO results.
- UTME printout and the corresponding result slip.
- Change of Course/Institution Slip (where relevant)
- Certificate of State of Origin, certified by a relevant authority.
- Birth certificate/affidavit.
